Heart failure: hospital to community

Of the 71,500 people in Australia who were admitted to hospital for heart failure in 2019, 65% had an emergency department visit within one year and 26% had a visit within 30 days, suggesting that many people were unable to manage in the community, an Australian Institute of Health and Welfare report has found. Hospital to Community: How Australians with Heart Failure Receive Health Care examined patient pathways for a year after hospitalisation. The report also found 24% of people died within one year of hospitalisation, with 41% of those being due to cardiovascular disease.
